Information Actions and Provisions to Support the Patient Journey Report
The report will be presented at the Public Health Conference 10th & 11th November 2011.
Background
The national Improving Equalities Data Monitoring Action Group facilitated by NHS Health Scotland and ISD supports NHS Boards in the gathering, sharing, analysing and interpreting of equality information. The work is divided into three areas:
- Improving ethnicity data collection, monitoring and using equality information
- Communication that has produced patient focused information about giving additional personal information to the NHS
- Identifying a List of Actions and Provisions to support the patient pathway.
The Information Actions and Provisions to Support the Patient Journey Report 
[100KB] was developed in partnership with NHS Boards, some service users and some voluntary groups to identify a comprehensive list of actions and provisions to support patients and assist health service staff to record and share information in a consistent way about the support needs patients have when using health services. Read Codes
NHS Health Scotland are still progressing the collection of equality data to support the patient journey. Read Codes 
[40KB] have been developed to support the findings of the Report.
Currently three Health Boards are exploring the usefulness of the Read Codes with some of their primary care colleagues and will feedback to the national Improving Equalities Data Monitoring Action Group
