Analytical Outputs

ISD has a range of linked data sets which are transforming our ability to understand the patient/client pathway. The linkage achieved in the context of the Integrated Resource Framework (IRF) allows health and social care activity to be costed at an individual level. We have used this intelligence, working with NHS and Local Authority Partnerships, to evaluate individual pathways across health and social care services.

In order for Health and Social Care Partnerships to commission and deliver services that best meet the needs of local communities and to intervene at an early stage to address health problems, they will require a clear understanding of the health and care needs of their population, from both the perspective of the NHS and Local Authorities.

Guide to Data

For each topic area, the main data sources currently available have been identified, as well as data resources currently in development. A description of what each data resource provides, supporting information to assist in using the data, and signposting/weblinks to where to find the data (and further guidance) is available.

It outlines the general approach to needs assessment and gives examples of data sources that could support each stage of the process.

Service Redesign, Integration and Strategic Commissioning

To support partnerships in service redesign, integration and strategic commissioning, we have developed a range of analytical outputs. See Standard Outputs and Developmental Outputs pages.

Bespoke Analytical Support

We also support the development of bespoke analyses as it is not possible to provide standard outputs that cover all areas of interest. This support ranges from evaluating a specific intervention such as anticipatory care planning to model whole system change which can enable the redirection of acute flow to social care settings.
