Primary Care Activity

The main source of primary care information for heart disease at present is prevalence data taken from the Quality and Outcomes Framework (QOF) disease registers.

In future it is anticipated that detailed information will be available from the Scottish Primary Care Information Resource (SPIRE). This supersedes the Practice Team Information programme which had collected data on CHD and angina from a sample of practices on number of consultations with GP or practice nurse up to 2012/13.

The QOF measures a GP practice's achievement against a set of evidence-based indicators and includes the collection of prevalence data in practice 'registers'. A QOF register may count patients with one specific disease/condition or it may include multiple conditions. Prevalence data is recorded for CHD, heart failure and atrial fibrillation.

Key points:

  • 4.1 % of all patients registered with a practice in Scotland were on the CHD QOF register in 2015/16, a total of 230,949 patients (QOF Table)
  • Smaller numbers of patients were on the atrial fibrillation and heart failure registers for 2015/16: 96,367 (1.7%) for atrial fibrillation and 47,806 (0.8%) for heart failure (QOF Table)
  • Trend data for prevalences has shown the prevalence of CHD to have decreased slightly over the last 10 years from 4.5% to 4.1%. On the other hand, atrial fibrillation prevalence has increased slightly from 1.3% in 2006/07 to 1.7% in 2015/16. Heart failure prevalence has remained unchanged over this period - 0.8% in 2006/07 and 0.9% in 2015/16.
