Prescriber Codes
The eVADIS Team within ISD maintains a detailed database of all NHS Scotland Prescriber Codes for prescriptions dispensed in the community in Scotland.
GP Prescriber Codes and GP Practice codes are issued by the Primary Care departments of the local NHS Health Boards and potential prescribers in this area should in the first instance contact their host Health Board.
The eVADIS Team maintains and issues codes for the following prescriber types:
- Supplementary / Independent Pharmacist Prescribers
- Community Practitioner Nurse Prescribers and Independent/Supplementary Nurse Prescribers
- Non-Medical Prescribers (Allied Health Professional Prescribers)
- Hospital Prescribing (using HBP, HBPN, HBPP and dispensed in the Community)
- Private Prescribers of Controlled Drugs (CDs)
Supplementary / Independent Pharmacist Prescribers
Requests should be completed on form ISD (P)3 v3

Community Practitioner Nurse Prescribers and Independent/Supplementary Nurse Prescribers
Requests should be completed on form ISD (P)1.1

Non-Medical Prescribers (Allied Health Professional Prescribers)
Requests should be completed on form ISD (NMP) 1
[48kb] and signed by an authorised Health Board signatory.
Hospital Prescribing
Requests should be completed on form ISD (HBP) 1
[44kb] and signed by an authorised Health Board signatory.
Private Prescribers of Controlled Drugs (CDs)
Requests should be completed on form ISD (PPCD) 01
