This project is in the early stages of development and has resulted from recommendations given in the NHS QIS report 'Safe Today - Safer Tomorrow: Patient Safety: Review of Incident and Near-Miss Reporting' (2006). The report recommended the development of a more coherent national programme for reporting on and learning from incidents in Scottish hospitals. This programme should facilitate incident reporting, support learning and sharing of best practice in relation to safety solutions. A pilot project to examine the reliability and comparability of critical incident data was completed in early 2010. Work is currently underway to define 'sentinel events' and plan for the extension of the project across Scotland as part of the Quality Measures Framework.
