SMR Completeness
A new patient management system (PMS) is currently being implemented in a phased approach across 5 NHS Boards (Borders, Grampian, Lanarkshire, Ayrshire & Arran, Greater Glasgow & Clyde). There has been a noticeable impact on the submission of SMR returns and therefore data should be interpreted with caution.
The table below summarises the current impact on SMR submission (as at 11th April 2012).

Note: The newly formed Data Support & Monitoring Team continues to work with Boards to improve the quality of data following go live on TrakCare.

NHS Boards are working to resolve these submission issues. ISD will continue to monitor the situation and provide regular updates on progress made.
Definition of SMR Completeness
NHS data providers will know how complete their SMR submissions are and the extent of any backlog. SMR data is expected to be received by ISD 6 weeks (42 days) following the end of the month of discharge / clinic date. ISD calculates backlog as data which is received after 6 weeks following the end of the month of discharge / clinic date.

Completeness Data Mart
A Completeness Data Mart had been developed which contains completeness information from January 1981 to present day on hospital episodes for:
- Outpatients (SMR00)
- Acute/general discharges (SMR01)
- Maternity discharges (SMR02)
- Mental health discharges (SMR04)
Additionally completeness information is provided from the following two sources:
- Cancer registrations (Socrates)
- Death registrations (GRO)
The information is provided in a series of standard reports. Data completion varies from month to month and the information provided in the data mart is to inform users on the completeness of data available to them in other data marts e.g. ACaDMe. All registered ACaDMe and Outpatient data mart users will have automatic access to the Completeness Data Mart. Other users e.g. SMR02 analysts can request access to the data mart.
