e-form workshop definition of Medicines Reconciliation:
“The process of obtaining an up to date and accurate medication list that has been compared with the most recently available information and has documented any discrepancies, changes, deletions or additions resulting in a complete list of medications accurately communicated.”
Further Information;
This CHH Bundle is a representation of the Greater Glasgow e-medicines reconciliation form, which has been agreed for adoption by a consortia of  NHS Scotland Territorial Board representatives that attended a series of  e-forms workshops. The adoption of this e-form was agreed by all of the attendees at the third e-forms workshop held on Thursday 3rd November 2011 at the Lighthouse, Glasgow.
The information captured within the form has been mapped to the NSS Compound Healthcare Headings (CHH) Model V4.0 and related CHH bundles. Clicking on the links will take you standards which can underpin your data and also defines where in the CHH structure the information within the e-form aligns.
