This CHH Bundle is a representation of the
Greater Glasgow e-medicines reconciliation form, which has been
agreed for adoption by a consortia of NHS Scotland
Territorial Board representatives that attended a series
of e-forms workshops. The adoption of this e-form was agreed
by all of the attendees at the third e-forms workshop held on
Thursday 3rd November 2011 at the Lighthouse, Glasgow.
The information captured within the form has been
mapped to the NSS Compound Healthcare Headings (CHH) Model V4.0 and
