This report provides an analysis of NHS smoking cessation services uptake and outcomes during the financial year 2015/16 and trend data for 2009/10 to 2015/16. Performance of NHS Boards against the 2015/16 LDP Standard will be included. The information presented is taken from the agreed national minimum dataset for smoking cessation services, developed by PATH (Partnership Action on Tobacco and Health), part of ASH (Action on Smoking and Health) Scotland. Dementia Post Diagnostic Support: NHS Board Performance, Dementia Post Diagnostic Support: NHS Board Performance 2014-15

Improving post diagnostic support has been central to The Scottish Government’s first two National Dementia Strategies. Commitment 2 of Scotland’s second National Dementia Strategy (2013-16) states that “We will transform the availability, consistency and the quality of post-diagnostic support by delivering the new post-diagnostic HEAT target”. The HEAT target (now LDP Standard) sets out the commitment:- ‘To deliver expected rates of dementia diagnosis and by 2015/16, all people newly diagnosed with dementia will have a minimum of a year’s worth of post-diagnostic support coordinated by a link worker, including the building of a person-centred support plan’ and came into effect from April 2013. This publication reports on NHS Board Performance regarding achieving this LDP Standard. This report will show performance related to people who entered the post-diagnostic service (PDS) between 1 April 2014 and 31 March 2015.
