Coding Guidance

Including a list of many frequently used and difficult to find codes in our Read code dictionary and Read coding guidance. You will also find the Nursing Activity Code list (formerly known as Appendix 3), a comprehensive list of nursing codes recommended for use in PTI recording. 

Read code guidelines 

PTI Intuitive Activity Codes - A shortened version of the Nursing Activity List showing only those activity codes which can be recorded without or in place of an associated morbidity. The codes are intuitive of the morbidity and allow quicker and consistency of recording. Please note however the Chronic Disease Section within the list can only be used with the provision the associated morbidity had been recorded in the patients record previously.
