Requests for Release of Patient Identifiable Information

Members of the Privacy Advisory Committee (PAC) receive copies of any applications for the release of patient identifiable information. The Caldicott Guardian for the Information Services Division ISD, or the Medical Advisor to the Link opens in new windowGeneral Register Office for Scotland (GROS) attach a covering explanation and proposed response to each.

PAC respond, in writing, to the Caldicott Guardian or Medical Advisor within four weeks. The Caldicott Guardian or Medical Advisor then decide if the release of information is permissible. At the twice-yearly meetings of PAC, members discuss requests that raise major issues of concern, or requests where PAC members have differing views. The Caldicott Guardian and Medical Advisor also attend these meetings.

You must submit an application to ISD or GROS:

  • for the use of Scottish Morbidity Records (SMR's) in medical research studies where any release of information by ISD will identify patients; or
  • for the linkage of previously unlinked records.

Use the following form and guidance notes for applications to ISD or GROS.
