2004/05 Achievement data at practice level - individual indicators

Click on the links below for excel tables containing data at practice level for individual QOF indicators, 2004/05, Scotland.  The data are grouped in two ways and can be accessed either by indicator domain / domain area or by NHS Board area. 

Please note: 

• The layout and presentation of the data files linked to on this page were revised on 11th July 2006, for improved clarity and compatibility with the equivalent data published for 2005/06.
• Some of the files are large and may take time to download.
• It is essential to take note of each practice's contract type.  In particular, practices with section 2c or 17c contract types may use only some of the QOF indicators, as part of their own, individualised, quality framework.  Where this is the case, recorded achievement under QOF may appear relatively low, but this should not be taken to mean that such practices are not providing a high level of service to their patients.
• Data are shown for those practices whose points and payments were signed off (or finalised prior to sign-off) at 25th August 2005.  QOF achievement points and payments remain to be agreed for some practices.  Additionally, it is possible that further small adjustments may be made for practices whose data have been signed off.
