Long Term Conditions Programme

The programme will support the agenda laid out in the Link opens in new windowBetter Health, Better Care Action Plan in the areas of long-term conditions, unscheduled care and their inter-relation.

High-Level Objectives 2008/09

  • Contribute to the risk stratification of the population of Scotland (at national and local levels) in terms of person-based patterns of long-term conditions
  • Develop sources of data and analyses to support the Long Term Conditions Collaborative
  • Continue to develop risk prediction tools as support for the better management of long-term conditions
  • Analyse the system of unscheduled care paying particular attention to heavy users, end-of-life care and relation to long-term conditions

Programme Summary

In the short to medium term, priority tasks for this Programme are focussed around high-risk complex patients - e.g. Scottish Patients At Risk of Readmission and Admission (SPARRA) risk-prediction tool - and their relationship with the acute sector. However, the programme is also working to support the information requirements of CHPs and to map out a population distribution of long-term conditions.

In the longer term the programme will continue to expand the information infrastructure to support the population management of long-term conditions both in terms of active risk stratification of individual patients and population level planning.
