Clinical Coding Guidelines

Clinical and health related data are collected on various national patient based datasets. To ensure data is recorded consistently and accurately, nationally agreed coding systems are used. The coding systems used are the International Statistical Classification of Diseases and Health Related Problems, Tenth Revision (ICD10) for diagnoses and the Office of Population Censuses and Surveys, Fourth Revision (OPCS4) for procedures.

To support of these nationally agreed coding systems, ISD have provided clinical coding staff with clinical coding guidelines since May 1996. This publication contains clinical coding advice as well as information about projects that are ongoing.
