SMR Training Aids

Electronic copies of the redundant paper forms on which SMR data was historically submitted are available below.  It is intended that these are to be used as an aid towards visualisation of the various SMR datasets.  Please note that it is not possible to submit SMR data in paper forms, all SMR data must be submitted to ISD electronically.
