Quarterly update of dispensing contractor's remuneration


INTRODUCTION

The tables display payments made to Scottish dispensers of pharmaceuticals in the community, up to December 2007.  They are intended primarily as reference tables but may also be used to compare trends of payments from month to month.

Tables show the remuneration costs and reimbursement amounts for financial year 2007/08 (up to December 2007) and calendar year 2007.

Each worksheet contains the overall Scotland total payments made to dispensing contractors, split between three defined groupings: community pharmacies, appliance suppliers and dispensing doctors.

KEY POINTS

  • In accordance with the new community pharmacy contract, remuneration payments made in respect of various fees have been subject to a general uplift of 2% for 2007-08.  Full details of the revised arrangements are given in circular PCA(P)(2007)10.
  • Compared to calendar year 2006, the 2007 calendar year gross total increased by 2% (£21M) to £1.06 Billion.  The increase was attributed to £7M net drug costs (NIC), and the remaining £14M increase was attributed to fees and allowances.
  • Of this gross amount paid, 84% (£888M) was attributed to drug reimbursement, and 16% (£176M) for the remuneration of services.
  • The total charges made to patients in calendar year 2007 were £40M; a 7% increase on charges made in 2006.

INTERPRETATION

  • When making comparisons from 2007 to 2006 you should be aware that
    • the generic discount was set at 0% from April to June 2006 instead of the standard 13.25%.
    • stoma and model schemes were introduced in April 2006.
    • unscheduled care was introduced in April 2006 and April, May and June 2006 fees were paid in June 2006 payments.
    • minor ailment service (MAS), public health and infrastructure were introduced from July 2006.
    • from November 2006, there was a revised list of zero discounted drugs.
  • Standard dispensing fees, instalment fees and professional allowance fees are only paid to community pharmacies not receiving transitional payments.  These fees show an increase of 58% from 2006 to 2007 because there is an increase in contractors not in the transitional payment scheme.
  • From April 2007 onwards, MAS payments have been subject to an uplift of approximately 2% (see circular PCA(P)(2007)10, paragraphs 23 & 24 for more detail).

However please note that the uplift which should have been paid in April, is reflected in the May payments, May uplift was paid in June and the payments for both June and July were paid in July.  From July onwards, the 2% uplift is included in the MAS capitation payment.

  • Public health service payments from April to June reflect payments for Tier 1 only.  The Public health service Tier 2 payment was implemented in July 2007 (see circular PCA2007(P)16 for more details).  From July 2007 onwards, Public health service payments reflect the total payments made for both Tier 1 and Tier 2.
